If all the parts look nice and clean and you are not getting water to brew through the grouphead, then most likely the pump is weak and needs to be replaced. The espresso is coming out too quickly, too watery and with little or no crema. You need to install the descaling tube in place of the milk container. Source

Towards the end (before I replaced and upgraded) I found that I got much better steaming and frothing results by removing the outer shell of the steam wand.
